Homebound Services are instructional services provided to students who are unable to attend school for an extended period of time due to medical reasons. Criteria for eligibility to be considered for homebound services are:
1) The student must need to be confined to home/hospital for a period of time totaling at least 4 weeks during the school year.
2) Confinement at home or in the hospital must be for medical reasons.
3) The student's medical condition must be documented by a licensed
physician (medical doctor).
For students in general education - the campus Section 504 Committee determines eligibility for homebound services & the need for a Section 504 plan. The committee must consider the physician’s information; however it is not the sole determining factor in the decision-making process. If your student is in general education and you need to request homebound services, please contact the school counselor.
For students receiving Special Education services - the ARD/IEP Committee determines the need for homebound services. The committee must consider the physician’s information; however it is not the sole determining factor in the decision-making process. If your student is receiving Special Education services and you would like the ARD/IEP Committee to discuss the possible need for homebound services, please contact the diagnostician assigned to your student’s campus.
