Employee Health Clinic

We are located at 701 South Bois D' Arc Forney, TX.  The entrance (door #1) is on the south side of the Administration Annex Building facing the baseball fields.

New Patient          Returning Patient

All visits are by appointment only.

Returning patients can self-schedule an appointment by using your Follow My Health patient portal.
Alternatively, you may use one of the options below:

• Call us at 469-762-4128 and press Option 1 to schedule an appointment

• Email us at EHCAppointments@forneyisd.net

• FollowMyHealth account to schedule an appointment immediately

Returning Patients:

Schedule a visit through the FollowMyHealth and login with the credentials.

1. Sign in to FollowMyHealth or create a new account

Choose “Sign in and add this connection”
 

If you have any questions, please call the EHC at 469-762-4128 and press option 1 for the office manager.

New patients: 

1. Create a new account

Choose “Sign Up and Connect” to create your FollowMyHealth username and password. 

(An email will be sent to confirm your account before logging into your portal to complete the forms)

2. Connect your account to Forney ISD Employee Health Clinic

Follow the on-screen prompts to connect your FollowMyHealth account to Forney ISD Employee Health Clinic. This includes entering the invite code. The invite code is the patient’s date of birth in the format MMDDYYYY (ex. July 9, 1985 would be 07091985) unless we gave you a different code.

Once connected select the patient you want to view from the Home screen.

Once you have appointment request has been approved you will receive an email from FollowMyHealth confirming your requested time.

You can also:
- Communicate securely with your physician
- View test and lab results
- Complete forms before your appointment
- Schedule and manage appointments

sPOUSE & dEPENDANTS

If you do not have a FollowMyHealth portal for your spouse or for your dependent, please include the following in the "Reason for Visit" section:

• Patient's date of birth

• The reason for the visit (e.g., Fever, UIL Physical)

*The appointment will be created for the patient and an invitation link will be submitted.

Minors

Appointments must be submitted by the legal guardian and accompanied by an adult with valid ID.  If the parent is unable to attend, we will need documentation that the minor may be seen by the adult present.

Minors: 2-15

Ages 16-18: Will need permission from legal guardian through the appointment site to be seen alone.  We will still need a valid ID.

UIL Physicals

An appointment will need to be scheduled for UIL Physicals prior to their visit. This is available for employee dependents only.

Athletics Physical Form

* Please bring the form with your child with the first page completed prior to the scheduled visit.
