Visiting the Clinic

With healthcare costs on the rise, Forney ISD offers a convenient resource to our employees. All full-time staff members, as well as their spouses and legal dependents from ages 2-26, are invited to take advantage of the district's free Employee Health Clinic regardless of insurance.

While not designed to replace primary care providers, a physician assistant or nurse practitioner will treat minor illnesses— sore throat, earache, sinuses, rashes, eyes, urinary problems, etc. UIL physicals are also available.

Everyone that visits the clinic must have the FISD employee's ID number each time they visit the clinic.

Weekly Schedule:

Monday 6:30am-3pm

Tuesday 9am-5pm

Wednesday 6:30am-3pm

Thursday 9am-5pm

Friday 6:30am-3pm

On school holidays when the clinic is open, operating hours are 8:00am-4:00pm.

We are located at 701 South Bois D' Arc Forney, TX. The entrance (door #1) is on the south side of the Administration Annex Building facing the baseball fields.

If you are a returning patient, you can schedule a visit through the FollowMyHealth and login with the credentials that you created from your first visit.

If you have any questions, please call the EHC at 469-762-4128 and press option 1 for the office manager.

Once you have appointment request has been approved you will receive an email from FollowMyHealth confirming your requested time.

New patients: "Sign up" for your FollowMyHealth account.

(An email will be sent to confirm your account before logging into your portal to complete the forms)

sPOUSE & dEPENDANTS

When scheduling an appointment as a spouse or for your dependent, please include the following in the "Reason for Visit" section:

The reason for the visit (e.g., Fever, UIL Physical)
Forney ISD employee's full name
Child’s date of birth (required for dependents ages 2–17)
- Note: For minor dependents, please use the FISD employee’s DOB when submitting the appointment request.

Minors

Appointments must be submitted by the legal guardian and accompanied by an adult with valid ID. If the parent is unable to attend, we will need documentation that the minor may be seen by the adult present.

Minors: 2-15

Ages 16-18: Will need permission from legal guardian through the appointment site to be seen alone. We will still need a valid ID.

UIL Physicals

An appointment will need to be scheduled for UIL Physicals prior to their visit. This is available for employee dependents only.
